Company name, address, country, VAT. Sender on the left, consignee on the right — don't swap them.
Field 3: where goods go (no date — the delivery date is entered by the consignee after unloading in field 24). Field 4: from where and when.
Gross weight WITH packaging. Be specific about goods — not "products" but e.g. "wooden furniture, EUR pallet".
Field 13: temperatures, ADR, customs, special instructions. Field 16: your company as carrier + truck and trailer plate numbers.

CMR stands for Convention Merchandise Routière — the international road consignment note required for cross-border goods transport between states party to the Geneva Convention of 19 May 1956. It's evidence of the carriage contract, description of goods, and acceptance by the carrier.
A minimum of 3 originals: first (red) for the sender, second (blue) for the consignee, third (green) for the carrier. IRU practice adds a 4th black copy for archive or successive carrier. Each copy is an equally valid original, signed by sender and carrier at pickup.
Per Art. 6 CMR Convention: field 1 (sender), 2 (consignee), 3 (place of delivery), 4 (place and date of taking over), 5 (documents), 7–11 (number, packaging, nature, statistical number, gross weight), 15 (payment), 21 (established), 22–23 (signatures of sender and carrier). Others are optional.
The sender (or forwarder on their behalf) fills fields 1–15 and 21 BEFORE or during loading. The carrier at pickup fills/verifies fields 16, 17, 18, 22–23 (their signature). Field 24 stays empty — it's filled by the consignee AFTER UNLOADING, entering place, date, and signing with stamp. It's the key delivery proof — without signature in field 24 the carrier won't be paid.
